About
Up & At Em Recovery-to-Community Transition Program Moving forward in recovery can involve much more than staying connected to recovery services. Employment, education, transportation, healthcare, benefits, housing, identification, community connections, and everyday responsibilities can all affect long-term stability. The Up & At Em Recovery-to-Community Transition Program helps individuals in or connected to recovery build a practical plan for the next stage of their lives. Support may include connecting individuals with recovery resources, treatment and community programs, employment and workforce resources, education and training opportunities, transportation resources, healthcare and benefits information, housing resources, identification assistance, and other community supports. For residents of sober homes and other structured recovery environments, the program can help identify barriers early and create a transition plan for life beyond the current living environment. We work alongside existing recovery supports rather than replacing them. We are not a treatment provider, recovery coach, therapist, or medical provider. Our role is to help connect the pieces and make it easier to navigate the systems and resources that support stability. Who is it for? Individuals working toward greater stability and independence during recovery or while transitioning into the broader community. Our approach: Recovery → Stability → Opportunity → Independence The goal is to help you build connections, address practical barriers, and move toward a more stable and independent future.
