You may know what you need, but not know where to start, who to contact, what paperwork is required, or what to do when a referral does not work out. The Up & At Em Resource Navigation & Stabilization Program helps individuals identify their immediate needs and navigate the resources and services available in their community. Support may include connections to housing resources, benefits, healthcare, mental health and recovery services, employment and education opportunities, transportation, identification and documentation resources, treatment programs, community programs, and other appropriate services. We can help you organize your next steps, make referrals, prepare for appointments or applications, follow up on referrals, and identify additional options when barriers come up. Our goal is simple: help you move from needing help to getting connected with the right resources. This program does not replace a doctor, therapist, attorney, recovery coach, shelter, housing provider, or other professional service. We provide navigation, coordination, referrals, and community support to help you move forward. Who is it for? Individuals who are experiencing barriers to stability or need help navigating multiple community resources. How it works: Identify → Plan → Connect → Follow Up → Stabilize Support may be provided virtually, by phone, in the community, or through other appropriate arrangements.
